Virtual Work requests require the completion of either the ADA Request or Telework Request Form. These need to be submitted immediately even if you do you not have the supporting documentation in hand.  

• ADA Request is to be completed for employees with a personal medical condition(s) that places them in the “high risk” category for covid-19 per CDC guidelines. On your form that “I am at high risk for COVID due to being in a high risk category due to CDC guidelines." 

• Telework Request is to be completed for employees with: 1) a family medical need; 2) childcare concerns; and/or 3) general concerns for not wanting to return face to face.   

If you are making either of these requests, please be prepared to provide documentation by July 24, 2020.

• If you selected Personal Medical, you will receive a reminder email on July 16 that in order for their personal medical need to be considered, to submit your ADA Accommodation Request form to the Office of Equity and Employee Relations (EER) as soon as possible, preferably no later than July 24. 

• If you selected family medical need, childcare concerns or general concerns, you will receive a reminder email on July 16 that in order to be considered, they need to submit a Telework Request form to the Office of Equity and Employee Relations (EER) by July 24.

If your request is completed by this date, you may resign without prejudice. Refer to Regulation 4291-Resignation for definition of “without prejudice” and other information.
